Plan Year
The 2008-2009 Health+ plan year runs from May 1, 2008 through
April 30, 2009. While the plan renews each year on May 1, you
may enroll on the first day of any month.
Medical Plan Options
• Basic PPO: A traditional Preferred Provider Organization
(PPO) plan with lower rates and higher deductibles, out-of pocket
maximums, and office visit co-payments.
• Standard PPO: The highest quality PPO plan available,
where deductibles, out-of-pocket maximums, and primary care
physician visits are lower than with the Basic plan. The percentage
of Out-of-Network coverage is generally greater than with the
Basic plan.
• High-Deductible PPO: All benefits, including pharmacy,
are subject to a $2,000 (In-Network) deductible on this plan.
After the deductible is met, the plan covers 80% for In-Network
care. This plan is compatible with Health Savings Accounts (HSAs).
• Value Plan PPO: New in 2008, the Value Plan has migrated
from an indemnity network to a PPO network. This improvement
leads to greater availability of discounts for Value Plan participants,
as well as in- and out-of-network benefits.
Medical plan participants receive automatic benefits, including
a 24/7 Nurse Hotline, Disease Management, Smoking Cessation,
WebMD access, and the Healthy Extensions Discount Programs.
Dental Plan Options
• Basic Dental: Coverage on preventive care (e.g. cleanings),
basic services (e.g. fillings) and major services (e.g. dentures)
is offered.
• Dental Plus: Dental Plus includes Basic Dental features
with the addition of orthodontia coverage for dependents under
age 19.
Vision Insurance
Offered through Spectera, Health+ vision members are eligible
for eye exams, lenses and frames (up to $130) when they visit
in-network providers. In-network copays for eye exams and lenses
and frames are $10.
Supplemental Life
Health+ offers the opportunity to purchase supplemental term
life insurance to full-time actively at work individuals. Tests
and underwriting are waived when employees enroll when initially
eligible. In 2008, the NFA expanded the supplemental life insurance
plan. Employees electing this benefit have the option to include
coverage for a spouse or dependent.
Members Plus Savings Program – “Benefits
Your Way”
Your employees can select three, six or nine health and lifestyle
benefits from an extensive list. Dubbed “Benefits Your
Way”, Members Plus allows employees to choose access to
the services they need most, and the program returns $2 per
month per enrolled employee to participating NFA members.

and assistance once you are enrolled in Health+. The center
