NFA Group Health Care Plan


Health+ Highlights

• All salaried and full-time hourly non-crew members are eligible for all plan options. You determine “full-time” status.
• The Value medical plan, both dental plans and the vision plan are voluntary benefits that do not require employer premium contributions.
• All employees who work an average of 20 hours per week are eligible for the Value Plan.
• Health+ Basic and Standard Plans, as well as the High-Deductible Health Plan, require minimum employer contributions of 25 percent of all coverage levels of 50 percent of employee only coverage.
• The Health+ waiting period expires on the first day of the month following 60 days of continuous employment.
• Employees and participating family members can receive credit for any portion of the deductible they have met under their prior health insurance program.
• Employee contributions are made on a pre-tax basis. Federal filings are completed by Health+.
• The Health+ annual open enrollment process is administered in-house by knowledgeable NFA professionals.
• NFA members can begin Health+ on the first day of any month, allowing for 60 days from the receipt of completed enrollment paperwork to effective date.
